3CX prices by Simultaneous Calls (SC), not extensions, and caps concurrent calls per tier. At a 1:1 ratio — every extension able to be on a call at once — 3CX must be sized to match extension count in SC. VitalPBX One doesn't cap concurrent calls, so its price is set by extension count alone.
